1. Parties & Scope
Alpha Community Care is committed to ensuring every participant is treated with respect, safety, privacy, and dignity. We operate under the NDIS Practice Standards, the NDIS Code of Conduct, and the Australian Privacy Principles (APPs). Your rights are central to how we plan, deliver, and review supports.
2. Services Provided
We provide supports that are safe, respectful, person-centred and aligned with your goals and preferences. You agree to provide accurate information to ensure safe delivery of supports.
3. Fees, Pricing & Payments
All supports are billed under the current NDIS Pricing Arrangements. Invoices are payable upon receipt unless otherwise agreed. Cancellation fees follow NDIS rules.
4. Participant Responsibilities
Participants agree to: treat staff respectfully, provide accurate information, maintain a safe environment, and give reasonable notice for cancellations.
5. Service Changes & Termination
We may suspend or end services due to safety concerns, repeated breaches, funding issues, or changes in your circumstances. SIL/SDA requires 14 days notice unless immediate risk exists.
6. Privacy & Confidentiality
We comply with the Privacy Act 1988 and Australian Privacy Principles. Your personal information is managed securely and confidentially.
7. Quality & Safeguarding
We comply with all NDIS Practice Standards. Serious incidents must be reported to the NDIS Commission.
8. Insurance
We maintain Public Liability, Professional Indemnity, and Workers Compensation insurance.
9. Conflict of Interest
We manage conflicts transparently. Participants maintain full choice and control.
10. Limitation of Liability
Our liability is limited to the extent permitted by law. We are not responsible for losses outside our reasonable control.
11. Australian Consumer Law
These Terms comply with Australian Consumer Law. Any unenforceable clause is severable.
12. Amendments
We may amend these Terms. The updated version will always be available on our website.
