NDIS Community Nursing in Sydney

Community nursing under the NDIS can support people at home with things like medication support, wound care and continence — within the scope of our registered community nursing care (0114). Referrals come through our national intake team, with hubs in Docklands and West Perth.

Share the suburb, any health information you are authorised to share, and how nursing would sit alongside SIL or daily living supports already in place. Intake confirms whether nursing can be arranged near you before a start date.

What happens after you refer

1. Call 1300 380 692 or email admin@alphacommunitycare.com.au, or use Make a referral.
2. Share plan details, suburb and the support you need.
3. We confirm what can be arranged near you, and when.
4. Only then do we talk through agreements and a start date.
